What kind of accident is about to happen?
|
A clamshell set on a gantry is being used to load dirt collected underground to a truck.
While the signaler is signaling the clamshell operator with a wireless device, he is also signaling a basement level bulldozer by turning a revolving light installed near the clamshell on and off.
Can you predict what's about to happen?

|
The signaler inadvertently entered the turning radius of the clamshell, and was injured when he was crushed between the counterweight of the clamshell and the gantry rail.

Tips for preventing similar accidents
|
1. |
The signaler should stand in a location where he can always see the bulldozer. |
2. |
The switch for the revolving light should be installed in a location where it does not cause workers to enter into a prohibited area within the turning radius of a piece of machinery.

3. |
Firmly surround areas that are not to be entered with barricades, so that no one can enter accidentally.
|
4. |
Take such safety measures as attaching a rotation warning device on the counterweight section of the clamshell. |
