Any accident foreseen in this frame?
|
An operator worker is clearing wastes in a chemical plant, using a lift truck. The wastes are put in two containers with the size of 108.5 cm x 108.5 cm x 78 cm each, which are laid down flat on a bit slanting ground. The operator has put a heavier container on the lighter one, put them both on the forks of a lift truck that is parked transverse to the slope, and is about to raise them.
Now what accident do you foresee occurring in this frame?

This is the accident resulted!
|
When lifted up as high as 20 cm, the containers started to slide down sideways, hitting a fellow sweeper who happened to be nearby. He was killed, thrown under the sliding containers.

Tips for forestalling similar accidents

1. |
Set working processes in advance that prohibit lifting cargos with a lift truck, while parking it transverse to a slope.

Allow only a licensed and designated operator to operate lift trucks.
|
3. |
Adjust the distance between each fork tine to the optimum width according to cargos to be handled.
|
4. |
Avoid partial loading.
|
5. |
Put a lighter container upon the heavier one, but never the other way round.
