Any accident foreseen in this frame?
A worker has finished a job of replacing a fluorescent lamp at the ceiling by himself. He then used a raised pallet of a lift truck as a foothold and is just about to step down to the ground.
Now what accident do you foresee occurring in this frame?

This is the accident resulted!
While coming down holding to the mast of the lift truck, one of his feet accidentally touched the lift lever, which started it to descend. He was guillotined between the inner and outer masts.

Tips for forestalling similar accidents
1. Through the basic training and education for safety, drill a simple principle into the operators and workers that they must not step in any other part of the lift truck than the operator's compartment.
*Never put a hand, foot or any other part of the body in the mast of lift trucks. It may be caught in a moving part of the mast, causing a serious bodily injury.
*Do not use the mast of lift trucks as a substitute for a ladder. He/she may slip off the mast and fall off.
2. Observe the instructions set forth in the Article 151-11 of Ordinance on Industrial Safety and Health.
They are:
* Lower work equipment like a fork or bucket to the lowest position possible.
* Take precautions required of loading vehicles such as switching off the engine and applying a parking brake without fail to ensure their safety parking.
3. For jobs at higher elevations, be sure to use a special purpose lift truck (otherwise called an order picking truck), or other vehicles for high lift work.
