Any accident foreseen in this frame?
Using a lift truck, an operator worker is carrying a steel pipe for a smokestack with the size of 854 mm in diameter x 2,850 mm in length and 850 kg in weight. In the traveling course of the lift truck, there is a runoff discharging pit that is 450 mm in width x 450 mm in length x 300 mm in depth and covered with a lid made of 12 mm thick plywood.
Now what accident do you foresee occurring in this frame?

This is the accident resulted!
The left front wheel of the lift truck rode over the lid and the lid gave in under the weight. The tilted lift truck sent the steel pipe sliding down, and the pipe hit a nearby signalman and injured him.

Tips for forestalling similar accidents
1. Select and install an attachment in the lift truck that is the best suited to cargos to be transported. At the same time fasten the cargo to prevent it from sliding down.
2. Do not lift up the fork higher than required, when it is loaded.
3. Determine a safe traveling course in advance and have an operator travel a lift truck along it without fail. If it cannot be avoided for a lift truck to ride over an opening, make a thorough review over the safety beforehand.
4. A signalman will take a post where he/she can be easily seen from the lift truck and guide it, keeping a sufficient distance for safety.
