What accident do you anticipate form this picture?
Using a lift truck, an operator worker is about to carry a sack called a flexible container containing refractory materials so as to load it on a truck waiting nearby. The sacks are stacked up in three layers in a plant compound with the ground of uneven surface. The operator picks up one of those sacks on the top layer and backs up the lift truck with the fork inserted through the hanging loops of the sack and still raised high.
Now what accident will you anticipate from this picture?

This is the accident resulted!
As soon as the operator began to back up the lift truck, it was tilted to the left side on account of the uneven surface of the ground, thereby starting the sack to swing to the right and left. The lift truck overturned eventually. The operator suspected a danger and jumped out of the operator's seat to get away, but it was too late, as the head guard fell over him and crushed him to death.

Tips for forestalling the accident
1. Put flexible containers (sacks) on a pallet and move them for loading thereafter, unless absolutely difficult to do so.
2. Lower the loaded fork to the lowest position possible, when traveling.
3. If it becomes definitely necessary to use a lift truck in combination with a hanging rope or a hanging belt, use the special attachments such as a crane attachment and a fork hook attachment.
4. Check the ground condition beforehand where a lift truck is used. Take necessary precautions like leveling the ground surface, slowing down the vehicle, turning the vehicle with great care, etc. in order to prevent the sacks from starting to swing.
