What kind of accident is about to happen?
A forklift operator is using two aluminum gangplanks to load a forklift onto the bed of a truck at a materials depot.
Can you predict what's about to happen?

Look what happened!
When the front wheels of the forklift were loaded onto the bed of the truck, one of the gangplanks slipped and the forklift suddenly tilted over. When the operator tried to get out, he tumbled over the side of the truck with the forklift and was pinned beneath it.

Tips for preventing similar accidents
1. Gangplanks should be placed securely at an appropriate incline.
2. Use the truck's installed winch or self-loader to load equipments.
3. Assign a worker to serve as a guide during loading.
