Runaway lift truck hits sweeper.
A mechanic was operating a battery-powered lift truck for the test after finishing repairs on the malfunctioned controller. The truck started to run away all of sudden and hit a nearby sweeper, causing her an injury of three months. Wrong wiring he made by mistake was to blame for the accident.



Corrective Action
1. After repairs on controllers, be sure to check if the commutation failure protection circuit functions properly.
2. When conducting a test operation, keep a wide area, put up a fence to keep bystanders off as well as post a work conductor.
3. Start a test run at a low speed and bring it up to a high speed, applying load in several stages.
