| Falling
bucket lands on inspector's head, injuring him seriously.
|
| Two mechanics are
checking the mast of a lift truck in pair with a bucket lifted up to the
uppermost point. One of them accidentally touched the bucket control lever
and the bucket was let fall all of sudden. The other mechanic was hard hit
by the falling bucket and seriously injured. |

| Corrective Action |
| 1.
|
If the bucket
must be lifted up for the work, either secure it with a rope or support it
with a block or pillar. |
| 2.
|
If the work
is to be done in pair, be sure to determine signals and have a safety
meeting beforehand. |
