| Malfunctioned jig
hurts assembly worker. |
| A worker was
press-fitting a sprocket shaft into the final drive housing of a
bulldozer. One of the support legs of the press-fitting machine that was
fixed to the housing went off all of sudden and hit him hard on the thigh.
He was seriously injured then. . |

| Corrective Action |
| 1.
|
Thoroughly
check not only the jigs and tools, but also the installation positions for
any damage or abnormality, prior to their use. |
| 2.
|
While press
fitting parts, do not get near the press-fitting machine nor touch it.
