A rock crusher wobbled and crushed a worker's ankle, causing a sprain.
As a rock crusher that had been repaired was being loaded onto the bed of a truck to be transported away, it became unbalanced and fell over onto the worker's ankle, causing a sprain.



Corrective Action
1. Only loosen a slinging wire after ensuring that a load is stable.
2. Perform work only after discussing work procedures in advance.
3. Use a base or ties to ensure that loads are stable.
