A worker's foot slipped from the tire he was using to climb down from a truck bed, and he fell and injured himself.

During bucket replacement work, a worker completed a task on the bed of a truck and started to climb down.
When he shifted his weight to his right foot, which he had placed on a tire, his foot slipped, causing him to fall and hit his left foot on the side of the bucket and the guard rail, injuring himself.



Corrective Action
1. Do not use tires to climb onto or down from vehicles.
2. Use a proper foothold to climb onto or down from a truck bed.
3. Increase the number of footholds from one to four.
