A worker was exerting his full strength to loosen an adapter on a hydraulic hose. When the adapter suddenly gave way, his momentum caused him to fall from the boom.

A worker climbed onto the boom of his vehicle to replace the hydraulic hose on the arm cylinder. While the worker was exerting his full strength to loosen the adapter of the hydraulic hose using a lug wrench covered with a pipe, the adapter suddenly gave way. His momentum caused him to lose his balance, and he fell backward off the boom and was injured.

Corrective Action
1. CLower the position of the boom to avoid performing unnecessarily elevated work.
2. If work must be performed while the boom is raised, create a place to stand on, such as an aerial work platform, so that you are not situated in an unstable position on the boom. Also use a safety belt.
3. Select a pipe suitable for the size of the lug wrench handle, and be sure that it fits properly all the way to the base of the handle.
