[Description of the Accident]

The victim was killed by electric shock when he touched the high-voltage transforming equipment at an auto-repair garage.

This workplace carries out the sale and repairs of automobiles. Although the accident occurred during the summer vacation period at the workplace, two to three employees had been taking turns coming to work to take care of their customers. On the day of the accident, the victim, who was a member of the service department, and another employee, who was in the sales department, were at work.

The victim, who was already at work when the co-worker arrived, was handling the switch of the air conditioner in the office, and mentioned that the air conditioner was not working.

As the co-worker then went to the showroom, and the victim went to the maintenance shop, there are no eyewitnesses to the victim's activities after that.

A short time later, all of the lights throughout the entire workplace suddenly went out. When the co-worker went to look for the victim, he found the victim lying on his back in the space between the high-voltage power-receiving facilities located on the north side of the office and the office building.


[Causes supposed]

1. While the victim had a wealth of knowledge and experience in automobile maintenance, he evidently tried to operate the switch even though he had no knowledge of power-receiving facilities or electricity.

2. Because the desk where the key to the power-receiving panel was kept had been left unlocked, the victim was able to freely take the key from the desk.

3. No safety education had been provided concerning the hazards of electric shocks, etc.
