Worker crushed to death by an iron plate that was dislodged by pile driver crawler

[Circumstances of Incident]

This accident occurred during elevated railroad construction work for the Shinkansen bullet line.

The project was being carried out by a joint venture for which the victim's company has been engaged in temporary earth retaining work as the primary subcontractor.

On the day of the accident,  three workers from the subcontractor were to bring a pile driver, its components and temporary structural materials into the work site, to prepare for H beam driving and sheet pile jacking operations  that were expected to start five days later using the pile driver. In the morning, they made arrangements for locations where bridge piers were to be built and laid three iron plates over the soft ground.

In the afternoon, the pile driver, the necessary components and materials were unloaded from a trailer truck. An operator moved  the pile driver onto the iron plates that had been laid in position in the morning and stopped it on the plates. Before moving to another location, the operator began to discuss the day's process with two other workers.  

When the meeting ended at around 14:00, the operator began to turn the driver to the right to move it to another space at the site. A worker who was providing guidance signals to the operator heard a dull thud. He stopped the driver and went to the back side of the driver and found a worker lying under an iron plate.


[Causes]

The direct cause of this accident was an iron plate that fell on the worker after being dislodged by the crawler of the pile driver during  the machine's movement. The following can be cited as underlying causes.

1 No signs or marks were posted to limit worker access to the site.

2. The pile driver was turned without signals or confirmation of the safety of other workers.
