Hit by an express train while painting a protective fence at a crossing barrier

[Circumstances of Incident]

This accident occurred when a worker was engaged in painting a protective fence at a crossing barrier. The worker entered inside the crossing and was hit by an express train.

The crossing where the accident occurred is located about 50m from the station, and trains stopping at the station can be confirmed from this crossing as the track is installed in almost a straight line.

As a crossing barrier with an alarm signal is installed at this crossing, all three workers including the on-site chief began painting the four protective fences for the crossing barrier without assigning a guard to watch for trains.

When one worker completed the work for the allocated protective fence, he crossed the track in order to help with work allocated to another worker. At this time, a train arrived at the station, and the alarm signal and crossing barrier began to operate. However, this worker passed under the crossing barrier and entered inside the crossing and was hit by an express train that approached from the opposite side.



[Causes]

The following can be considered as the causes of this accident.

  1. The worker entered inside the crossing despite the fact that the alarm signal and crossing barrier were operating normally.

  2. The worker mistakenly thought that the alarm signal and crossing barrier were for the train stopped at the station, and was not aware of the approach of the express train from the opposite side.

  3. Work was conducted without assigning a guard.

  4. Work was conducted without fully examining the procedures required in carrying out work in close proximity to the railway.

  5.  Safety education was inadequate.
