A hoist came off the rail when a ladle was being moved by a floor operated overhead crane, and a worker was covered with the molten metal

[Circumstances of Incident]

This accident occurred when a ladle was being moved by a floor operated overhead crane to pour molten metal into a mold. As a weld fixing the rail came loose, the hoist dropped from the rail.

On the day of the accident, the victim and a co-worker had taken turns in extracting molten metal from the melting furnace, moving the ladle by the floor operated overhead crane, and pouring it in the mold. They had already done these operations for about 10 minutes on that day.

Around 4:10 in the afternoon, a plant engineer who was checking the molten metal in the melting furnace noticed flames in the direction of the casting line, and went to that direction to discover the victim who was covered with molten metal and lying under parts of the crane amidst the flames.



[Causes]

The following can be considered as the causes of this accident.
  1. As a weld fixing the rail at the portion where a shift is made from the straight-line section to the curved line section had broken loose and the rail was hanging down, the wheel of the hoist slipped off the rail at the opening.


  2. Annual and monthly inspections for the overhead crane were not implemented.
