Insecticide spouted from a spraying nozzle during preparatory work after pouring insecticide into a sprayer tank

[Description of the Accident]

This accident occurred during preparatory work for insecticide spraying. The insecticide was sprayed into the face of the machine operator, the victim.

The victim, who was the grounds manager of a golf course, noted a new type of harmful insect during a patrol. In order to examine the effect of an agricultural chemical, the victim poured 20 litters of diluted insecticide into a tank mounted on a remodeled cart used for spraying purposes in a strage yard.

The victim then started the engine and was unreeling the hose by holding the spraying spout in his hand when the volume adjustment lever attached to the nozzle was accidentally opened by contact with the hose and other materials. A gush of insecticide was sprayed in the face of the victim, who swallowed some of the chemical.

While the victim took emergency measures such as washing his face and drinking water immediately after the accident, he became sick about 20 minutes later and went to a nearby hospital for medical examination. The victim was hospitalized for treatment of chemical poisoning.


[Causes supposed]

1. As the engine was started in a condition that the open/close lever of the sprayer would be either open or loosely closed, the insecticide entered the hose.

2. As the victim had handled the spraying cart only two or three times a year, unreeling the hose did not go smoothly, and the hose became entangled in the lever and hit against the spraying cart, and opened the lever.

3. When unreeling the hose, the victim held the spout with the nozzle towards his face.

4. The Course Management Work Safety Manual for the work of handling insecticide did not define the details for protective equipment and its usage methods.
