Worker killed in fall from forklift pallet

[Description of the Accident]

This accident occurred during work at a container yard to move containers from a freight train to a warehouse by forklift. Around 5:30 in the afternoon, it was found that a cargo container was one package short.

The victim went to a shed to look for the missing package. He found and transported it to the container yard by forklift. The victim then got on the pallet along with the package and asked the sub-leader to operate the forklift to carry him to the cargo container to replace the missing package.

After arriving at the container, the sub-leader started to raise the fork to a height of 2.5 meters and move the forklift forward to set the pallet on the roof of the container. The victim suddenly fell from the pallet and was fatally injured when he hit the floor.

[Causes supposed]

1. The forklift was used for other than the intended purposes i.e., attempted to lift the victim to the top of a container.

2. Despite the fact that considerable work needed to be done at elevated places, no equipment for stepping up safely was provided.

3. As the company employing the victim exclusively handled work for a major transport company, the work site was the premises of the major company and the work instructions were also dependent on the major company.

4. Although a safety supervisor had been appointed, no specific activities were carried out, and no manual, etc., for the work in the container yard was prepared.
