Victim crushed when machine slipped off a jack

[Description of the Accident]

This accident occurred during the work to transport five manufacturing machines to another factory.

On the day of accident the machine transportation work had been conducted by a total of six workers¾four workers from a company of special transportation and two workers from a subcontractor. Four out of the total of five machines had already been loaded on a truck by a traveling crane after being loaded on dollies and pulled into position by a forklift.

For the transportation of the fifth machine, two workers started slinging the load for the traveling crane and four workers started preparations to move the machine.

As the shape of this machine was not well-balanced to keep it stable, it was decided to use two rollers under one side. When the machine was about to be lowered to a bottom plate after being jacked up about 10cm, the load became unbalanced and slipped off the jack, and crushed a worker from the subcontractor who was monitoring the balance. The worker was immediately taken to a hospital, but died from internal injuries.


[Causes supposed]

1. Even for work to transport unbalanced load of large, heavy machine, no particular measures were taken to prevent a fall of the machine.

2. A jack was applied at a place that was not the machine's center of gravity.

3. The victim was allowed to enter the area existing possible dangers.

4. The workers concerned were not fully instructed concerning the transportation procedures of heavy objects.
