Crushed by falling bags piled in a warehouse

[Description of the Accident]

This accident occurred at the warehouse of a factory that manufactures plastic products. The victim was crushed when a pile of bags containing plastic pellets collapsed.

The victim was assisting the factory manager who was operating a forklift to deliver bags of pellets from the warehouse to the factory. When the manager was called away to the office, he instructed the victim to continue unloading bags of pellets from the forklift.

Around 4:00 in the afternoon, the manager wondered why the victim did not come into view and went to the warehouse to check. He found the victim crushed below a pile of fallen bags. Although an ambulance was immediately called and the victim was quickly taken to a hospital, it appeared that the victim had been killed instantly due to the rupture of internal organs when the bags collapsed on him.


[Causes supposed]

1. The bags of pellets were stacked in an unstable manner.

2. No measures had been taken to prevent the collapse of the piles.

3. No work plan was prepared for the work using a forklift.

4. The forklift was operated by a person without qualifications.
