Operator fell and killed when drag shovel toppled from road by collapsing shoulder

[Description of the Accident]

This accident occurred when a road was being cleared by drag shovel to allow timber to be taken from a mountainous logging site. The victim, the operator of the drag shovel, was killed when the shoulder of the road collapsed, toppling the shovel.

On the day of the accident, six workers entered the site at 7:00 in the morning. Following a meeting, each worker began working at the assigned site. After lunch, the victim continued the work to clear a road by drag shovel to transport the felled timber.

Around 2:00 in the afternoon, a worker who had completed his assigned work went to the road clearing site and found the victim lying face down by the side of the drag shovel, which has been overturned on the edge of the slope.

An ambulance arrived after about one hour, and the crew confirmed that the victim was dead.



[Causes supposed]

1. The shoulder of the road where the drag shovel was operating was soft.

2. The width of the road where the fall occurred was narrow (3.5m) compared to the overall width of the drag shovel (2.8m). This made it likely for one of the caterpillar treads of the drag shovel to ride on the shoulder.

3. Because of considerable snow at the site, it was difficult to check the situation around the shoulder of the road.

4. The victim was carrying out work at a site different from the assigned site.
