Drawn into the mixer while cleaning a concrete pump vehicle

[Description of the Accident]

This accident occurred as a worker was pulled into the mixer of a concrete pump vehicle by a rotating mixing rod while he was working to clean up the inside of the mixer.

On the day of the accident, the worker returned to the company office after making a pressurized concrete pour at a construction site. At about 2:30 PM, he was cleaning up the inside of the mixer of the concrete pump vehicle, which was parked at a location some 30 meters from the office.

At around 3:00 PM, a fellow worker found him dead in the mixer. A mixing rod is believed to have caught him and pulled into the mixer. At the time, the engine of the concrete pump vehicle was still running and the operating lever was at the activated position.

A mixer is a device to mix concrete delivered from concrete mixer truck in order to prevent it from hardening. A rotating mixing rod with blades is located in the mixer. Although a protective metal net is mounted on the opening of the mixer during pressurized concrete delivery, it is necessary to remove the net when the mixer is being cleaned.


[Causes supposed]

1. The victim was carrying out cleaning work without turning off the power switch of the mixer.

2. The worker was too close to the rotating mixing rod while washing the inside of the mixer.

3. The worker is believed to have put his hand into the mixer while the mixing rod was rotating.
