Caught in between a building wall and an aerial platform while operating the platform

[Description of the Accident]

In this accident, a worker was caught in between the wall of a six-story building under construction and a mobile-type aerial platform on which he was working. The worker had been carrying out concrete wall finishing work in the basement to mortar the traces of metal fittings that had been left on the walls after concrete placement.

On that day, five workers had been carrying out the concrete wall finishing work: a foreman, the victim and three other colleagues. Both the foreman and the victim were independently working on their own aerial platforms to carry out the mortar-covering work, with the other two workers working on and under another aerial platform. The fifth worker was working alone on scaffolding.

The work started at around 8:30 AM in the basement. The foreman left the work site to attend a meeting. After lunch, four workers had started working again in the same positions as in the morning.

When the foreman came back to the basement at around 2:00 PM, he noticed that the victim and his aerial platform had left the original working position. At around 2:45 PM, the severely injured victim was found by a worker of another company. The victim had been caught in between the upper part of a door opening and the handrail of the aerial platform. The victim was rushed to a nearby hospital but died on 3:30 PM on the same day.


[Causes supposed]

1. As the victim had not undergone special training for aerial platform operations (with a platform height of less than 10 meters), his knowledge and skills concerning aerial platform work was insufficient.

2. A work plan for operations using aerial platform had not been developed. In addition, a work supervisor for the operations had not been assigned.

3. The height of the aerial platform was too high for the dimensions of the wall opening through which the victim tried to pass through.
