Fatal heat stroke during work to process industrial waste

[Description of the Accident]
From the morning on the day of the accident, the victim was engaged in
work to sort scrap material brought in from a construction site at an industrial
waste processing site, and deliver the sorted material to the prescribed
waste dump.
As the victim started staggering around 15:00, a co-worker told him to take rest in the shade. When the victim started to walk toward the office, he suddenly crouched down and stopped moving.
The victim did not respond to his co-worker's calls, and did not drink the water his co-worker tried to give him.
Accordingly, the co-worker took the victim to a car located nearby, and let him lie down in the car after turning on the air conditioner. As there was no sign of recovery, the co-worker took the victim to the hospital by the same car.
However, the victim did not recover and died of heat stroke.

[Causes supposed]
1.The victim was doing work under the blazing sun with the temperature
exceeding 30 degrees C., but was not taking appropriate water and salt.
2. The victim was not wearing clothes appropriate for work under the blazing
sun (the victim was wearing a helmet with towel inside).
3. The accident occurred on the 5th day after the victim had joined the
company, and a health examination was not conducted at the time of employment.
Moreover, the victim's physical condition was not checked on the morning
of the accident.
