Fall during tiling the roof of a new house

[Description of the Accident]

This accident occurred during preparatory work for the construction of a new house for a private owner.

The work contents included installing corrugated tin sheets at the eaves of the first floor at the north side of the house by two workers. After a meeting, two workers climbed to the roof of the first floor and started working.

About 15 minutes after the start of the work, one worker noticed that he left his metal shears in his car, and moved to a portable ladder that was set against the east side of the house in order to retrieve them.

When he turned his head towards the victim who was on the roof of the first floor, he saw the victim near the skylight (about seven meters in height from the ground) of the second floor roof, which was not included in the work contents for that day. Immediately after thinking this was strange, he saw the victim fall through a sheet of styrofoam covering the skylight and land on the unfloored part of the house on the ground.



[Causes supposed]

1.Clear work instructions were not given.

2. Measures to prevent falling through the temporary styrofoam covering were not taken.

3. Safety and health education was not provided.
