Fall from a cable crane bucket during retaining wall construction

[Description of the Accident]

This accident occurred while using a cable crane to install concrete retaining wall.

The victim was inside the cable crane bucket that was used to carry freshly mixed concrete, and using remote-control operations by himself to maneuver the bucket along the wall to allow him to patch the cavities in the surface. As the bucket started swinging, he asked workers who were carrying out scaffolding dismantling work below him to steady the rope until the swinging of the bucket could be stopped. Once the swinging stopped, the other workers returned to their work and the victim continued with his work. At around 3:00 PM, the victim accidentally fell from the bucket, falling some 42 meters down the slope to his death.


[Causes supposed]

1. The victim got in the cable crane bucket, which was designed to carry freshly mixed concrete, and carried out the work without using an access platform or ladder.

2. No safety education concerning cable crane operations was provided.
