Jib collapses during disinfection work for fishes by using a crane-mounted boat at an aquaculture pond

[Description of the Accident]

During disinfection work for fishes in an aquaculture pond by using a crane installed on a boat, the bolt mounting of the lower frame that supports the revolving axis of the crane broke and the jib collapsed. Two workers who tried to jump out of the way were severely injured when they struck their heads and faces on the steel framework over the pond.

The work was to clean bacteria from the fish in the pond. The procedure consisted of putting the crane-mounted boat at the edge of the pond, using a net to gather fish to the side of the boat, cleaning the fish in sheeting filled with fresh water, and returning the fish to the pond by using the crane to tip the sheeting.

On the day of the accident, workers had just started to undertake work on the fourth pond after completing such work for three other ponds. As they rotated the crane, the bolt mounting that supports the revolving axis of the crane broke and the collapsing jib swung in the direction of the frame over the pond.

When two workers who saw this accident tried to jump from the frame over the pond into the sea, they injured their faces, etc., on the pond frames.


[Causes supposed]

1. Inspection and maintenance of the bolts mounting the lower frame that supports the revolving base and axis of the crane were not regularly implemented.

2. A lifting-load chart based on the angle of inclination of the crane jib and an overload protection device were not prepared.
