Large truck smashes into an elevated work vehicle being used to install roadway lighting fixtures

[Description of the Accident]

In this accident, a large truck traveling on a road crashed into an elevated work vehicle during work to install lighting fixtures on a road.

On the day of the accident, before installing the lighting fixtures, workers began removing tree branches that would impair the movement of the elevated work vehicle. While this work was going on, a 10-ton truck failed to heed the colored sign placed to prevent entry into the working area and crashed at full speed into the lower part of the bucket and the outriggers of the elevated work vehicle.

Because of this, the victim who was working in the bucket and the assistant foreman who was operating the arm of the elevated work vehicle were flung to the roadway, causing the victim's death due to a fractured skull.


[Causes supposed]

1. Not enough marker signs were positioned along the roadway to indicate the working area.

2. The truck driver entered the working site without slowing down.

3. The bucket of the elevated work vehicle was located at a relatively low position.

4. Methods to provide guidance to the passing traffic were not appropriate.

5. The elevated work vehicle was being operated by a non-qualified person.
