A worker is struck by a load lifting attachment on cargo hoisting equipment during work to unload lumber in hold

[Description of the Accident]

While working to unload cargo (imported packaged lumber) from the hold of a foreign vessel moored alongside a pier, the victim, a stevedore who was lining up the eyes of a belt sling that had been passed under the cargo bundle in the hold in advance in preparation for hoisting them out of hold, was struck by the descending load lifting attachment of the vessel's cargo hoisting equipment.


[Causes supposed]

1. The operator of the cargo hoisting equipment failed to confirm a sign by signaler before operating the equipment.

2. The signaler was issuing signs in a location that could not be seen from the driver's seat.

3. The preparatory slinging work was being carried out without coordination with the signaler.
