What kind of accident is about to happen?

The driver of a truck is leaning out of his window to pass documents to the operator of a hydraulic shovel with the engine running that is stopped next to it. Can you predict what's about to happen?

When the operator turned to take the documents, his clothing got caught on the control levers, causing the shovel to rotate. A worker who was standing near the back of the truck was crushed between the bucket of the shovel and the truck bed.
