Because a truck rail was not locked, a worker fell down and broke his wrist.

An empty drum was lowered onto a truck bed using a crane during a replacement of hydraulic oil. A worker put his foot onto the rail to remove the wire, but the rail opened and fell, breaking the worker's right wrist.


Corrective Action
1. Use a proper step to climb onto the bed of a service vehicle.
2. When using a crane to load and unload objects, people should work together from designated positions.
