A jack fell while being loaded into a van and injured a worker's foot.

A worker was loading a transmission jack into a van by himself. The handle of the jack slipped out, causing the jack to fall and injure his right foot.


Corrective Action
1. Use a machine, such as a reach forklift, to load heavy objects.
2. Use two or more people to load heavy objects.
3. Wear safety shoes with hard toed protection.
