Attached Table 5 Contents of information for use and how to provide it

  1. The contents of information for use shall contain the following and other directions required to ensure safe and correct use of the machinery which shall be indicated in notification or warning:
    • (1) The name and address of the entity engaged in manufacturing, etc.
    • (2) Information for identifying the model or manufacturer's serial number, etc.
    • (3) Information about machinery specifications and structure
    • (4) Information relating to use of the machinery
      • a) Intended use objectives and methods (including information about machinery maintenance and inspection, etc.)
      • b) Information about the onset of use such as transport, installation, test run, etc.
      • c) Information about de-commissioning such as dismantling and disposal
      • d) Information about machinery faults, abnormalities, etc. (including information about re-startingafter repairs, etc.)
      • e) Reasonably foreseeable misuses and prohibited usages
    • (5) Information about safeguarding and complementary protective measures
      • a) Objectives (target hazards)
      • b) Locations installed
      • c) Safety function and its schematic representation
    • (6) Information about the residual risks, etc. of machinery
      • a) Risks that could not be removed or reduced by using protective measures by the entity engaged in manufacturing, etc.
      • b) Particular risks which may be generated by certain applications or by the use of certain fittings
      • c) The contents of safeguarding, complementary protective measures, worker training, use of personal protective equipment, or other protective measures to be implemented by employers using machinery
      • d) Material safety data sheets for chemical substances handled or emitted by machinery in intended uses
  2. The methods of providing information for use shall be those specified below or other appropriate ones.
    • (1) Attach signs, warning labels, etc. as set forth below.
      • a) They shall be attached near potentially harmful parts, inside, on the side of, or on top of the machinery or at other appropriate locations.
      • b) They shall remain clearly legible throughout the expected life of the machinery.
      • c )They shall not readily come off.
      • d) Signs or warning labels shall be as set forth below.
        • (a) The types and nature of the harm shall be described.
        • (b) Prohibitions or obligations shall be specified.
        • (c) They shall be clear and readily understandable.
        • (d) They shall be able to be provided again.
    • (2) Warning devices shall be as set forth below.
      • a) Warnings based on audible signals or visual signals shall be used as necessary.
      • b) They shall be installed inside, on the side of, or on top of the machinery or at other appropriate locations.
      • c) Warning devices used to emit important warnings such as machinery start-up or over-speed shall be as set forth below.
        • (a) They shall foresee a hazardous event and shall be emitted before the hazardous event occurs.
        • (b) They shall not be vague.
        • (c) They shall be able to be clearly perceived and differentiated from all other signals used.
        • (d) They shall be warnings to which people will not easily become “sensorial saturation”.
        • (e) They shall be located such that checking is easy.
    • (3) Production of the instruction handbook and other documents shall be as set forth below.
      • a) They shall be provided when the machinery is delivered or at an appropriate time before that.
      • b) They shall be durable enough to remain legible until the machinery is scrapped.
      • c) They shall be written as briefly and readably as possible.
      • d) They shall be able to be provided again.
