What kind of accident is about to happen?
Wet concrete for a retaining wall construction project is being loaded into the bucket of a hydraulic shovel. The arm of the shovel, which is parked on the edge of a shoulder, is being extended to pour the concrete into the wall frame. A worker is standing on the lower edge of the frame.
Can you predict what's about to happen?

Look what happened!
When the shovel operator made a slight steering adjustment while trying to position the bucket properly, the shoulder collapsed, causing the front part of the crawler to tilt forward. The bucket then crashed into the worker who had been casting the concrete.


Tips for preventing similar accidents
1. Determine where to position machinery only after checking the strength of the edges of any shoulders.
2. Do not make steering adjustments while on the edge of a shoulder.
3. Confirm that no workers are in the way when adjusting the position of a machine.
4. Do not enter the operating range of a machine.
5. Assign a guide worker when necessary.
