Any accident foreseen in this frame?
A worker is walking along an aisle with a waste box in his hands in a particleboard manufacturing plant. He is cleaning the shop with earplugs on. A lift truck loaded with particleboards is about to catch up with him from behind.
Now what accident do you foresee occurring in this frame?

This is the accident resulted!
He suddenly stepped down to the aisle to walk around a pillar. As the lift truck operator had sighted him, he was cautiously driving the lift truck. He hit the brake to see the worker stepped down all of sudden. But a heap of particleboards on the pallet collapsed and fell over the worker.

Tips for forestalling similar accidents
1. Provide a sufficient width to the aisles in a plant so that the workers can walk along safely.
2. Instruct a lift truck operator to tilt back the mast of a lift truck sufficiently, when loaded.
3. Train the workers intensively for observing the basics for walking along the aisles without fail, such as check of the right and left directions, stand-by, etc.
4. Instruct the workers to remove all personal protective equipments like dust-proof goggles and earplugs, while walking.
5. In such a noisy workplace as to require earplugs, install a rotating yellow lamp on lift trucks that optically warns approach of the vehicle from a distance.
