Any accident foreseen in this picture?
A truck driver happened to find a reach lift truck (max. lifting weight 1.2 ton) unattended with an ignition key inserted. He is not licensed for operating lift trucks, because he has yet to finish the required training course. With this reach lift truck he is about to try to load cargos on a pallet from a platform on to his truck.
Now what accident do you foresee occurring in this frame?

This is the accident resulted!
The truck driver traveled the reach lift truck toward the cargos along the edge of the platform. But by mistake he drove the vehicle so close to the edge that it went over the edge, crushing him to death.

Tips for forestalling similar accidents
1. Keep the use of lift trucks under strict control, never allowing their unauthorized operation.
2. Set standards for the use of lift trucks that include the minimum space for use, necessary lift truck capacity, optimum cargo configuration, etc. and make sure that the operators will follow them without fail.
3. Provide a fall-off proof structure at the edge of a platform.
