Any accident foreseen in this frame?
A worker is storing commodities on the rack in a warehouse with the help of a lift truck. The warehouse is dimly lit, so that he can hardly see clearly around his feet. Now that he has been instructed to store them in each designated location, he is trying to climb on to the 3 meter- high rack from a pallet inserted into fork tines.
Now what accident do you foresee occurring in this frame?

This is the accident resulted!
Since he was standing on the edge of the pallet, it was suddenly tipped to one side, which threw him off the balance and let him fall off dead.

Tips for forestalling similar accidents
1. For jobs at higher elevations, be sure to use a special purpose lift truck (otherwise called an order picking truck), and also instruct the worker to fasten a safety belt without fail.
2. Light up the entire warehouse brightly enough even in the daytime, not to mention at night, to ensure a safe working environment, whenever a work is to be done in the warehouse.
3. Instruct a supervisor to prepare for a work plan which clearly sets forth the work procedures. Furthermore make sure that he/she will be available at the working site for necessary instructions, whenever there is any unexpected additional work, in order to ensure safety in the work.
