What accident do you anticipate from this picture?
Here you are in a container yard of a freight terminal. An operator worker is required to load the container A on a truck. To do that, he must first move a container right before him to a vacant lot on the left side. Meanwhile another worker is about to check a tag on the container B.
Now what accident will you anticipate from this picture?

This is the accident resulted!
The lift truck operator pushed the container forward, not knowing there was another worker on the other side. The latter was sandwiched between the two containers and wounded.

Tips for forestalling the accident
1. Make out a working schedule as well as working procedures beforehand so that lift truck jobs and other jobs will not be overlapped, and make them known to every worker concerned.
2. If such overlapping of the jobs cannot be avoided, post a signalman on the scene.
