Mechanic falls off machine while removing brake piping.
In order to replace the brake piping on wheel loader, a mechanic climbed on the rear wheel and was trying to detach the brake piping with a single ended wrench inserted into a pipe. The wrench slipped out of the impromptu extension handle, and his own force sent him down on the ground and injured him. As illustrated at right, the handle of his wrench was lengthened with an extension pipe whose tipped was flattened to hold it firmly. .



Corrective Action
1. Take steps necessary to facilitate the work such as dismounting the tire.
2. From the standpoints of safety keeping as well as observing the prescribed tightening torque, do not try to extend the handle of wrenches and spanners with a pipe in principle.
3. Use permeable lubricant, when dealing with bolts and nuts hard to loosen.
