| Sudden bucket
movement shakes off mechanic inside. |
| A mechanic stepped in
the bucket of a hydraulic excavator and was checking the pin securing the
bucket. Not knowing that, another mechanic operated the bucket control
lever. The bucket started moving and threw him off on the ground. He then
tried to protect himself with his hands and sprained the wrist. .

| Corrective Action |
| 1.
|
Before
starting the machine, pay attention to the workers around as well as the
neighborhood, and take safety precautions such as evacuating the workers
and others. |
| 2.
|
Stop the
engine while servicing the machine. |
| 3.
|
If it becomes
definitely necessary to start the machine during inspection or maintenance
work, appoint a work conductor and follow his/her instructions.
