When a worker tried to climb onto the shoe of a swamp bulldozer, he slipped and injured his knee.
At the end of a project, a worker tried to climb onto the track shoe of a bulldozer to drive it away from the construction site. While in midair, his foot slipped, and he cracked his knee on the shoe.

Corrective Action
1. Always use a step or handrail when climbing onto or down from a machine.
2. Before performing work, remove all sludge and dirt from machinery. When work is completed, clean off any oil or grease that has accumulated.
