When a socket wrench slipped off, a worker's momentum caused him to fall off a vehicle and bruise his hip.
A worker was using a socket wrench to loosen a bolt so that a counterweight could be removed from a hydraulic shovel. When the socket slipped off the bolt, the worker lost his balance and fell from the vehicle, bruising his hip.



Corrective Action
1. Ask another worker to help with this kind of work to keep socket wrenches from slipping off or wobbling.
2. When working with parts that require a lot of torque, or when working in an unstable location, use a torque wrench with a pipe extension.
