A worker's fingers were crushed between a crane hook and a shackle while a Vibrohammer was operating.

A worker was trying to transfer a Vibrohammer for repair work, but since the slinging wire was not around him, he attached the crane hook directly to the Vibrohammer shackle while operating the overhead crane himself. When he operated the crane hook， the hook started to come off, and the worker, without thinking, tried to support the hook with his right hand as the load was being lifted, causing his hand to be crushed between the hook and the shackle.


Corrective Action
1. Always use a slinging wire. Since a load will wobble if the hook is positioned too far away, as shown in the image, attach the hook directly above the load.
2. When hanging a heavy object that is difficult to balance, use at least two people to perform the work and raise the object slowly, while carefully monitoring its stability.
