A chisel to be attached to a breaker turned during crane slinging work, crushing a worker's fingers.

In the process of hanging a chisel from a crane so that it could be attached to a breaker, the chisel turned off its tie and started to fall. A worker tried to stop it with his left hand, causing that hand to be crushed between the chisel and the floor.



Corrective Action
1. When placing objects that can turn easily onto a tie, use rotation stoppers.
2. Strive to keep body parts out of the direction in which objects might turn.
3. Observe the fundamental rules of crane operations, and take safety precautions before performing work.
