While the bucket of a shovel loader was being repaired, it fell on a worker's left foot.

A worker was attaching teeth to a bucket that had been removed from a shovel loader and had it tipped up and balanced on its curved edge. The bucket became unbalanced and tipped over, and one of its teeth crushed the worker's left foot.



Corrective Action
1. When an object is in danger of falling over, brace it with blocks to prevent it from falling.
2. Warn one another when you notice someone engaging in an unsafe behavior (raise the level of safety awareness).
3. When working on unstable objects, perform work on the stipulated plate.
