A worker fell and was injured when a trucker’s hitch came untied.

A worker used a trucker’s hitch to secure hydraulic oil drums to a service vehicle with a rope. When he pulled hard on the rope, the head of the knot slipped, causing the worker to lose his balance, fall over backward, and land hard on his back.

Corrective Action
1. Tie a trucker’s hitch properly with a head loop 5 cm. in diameter.
2. Before pulling a rope at full strength, make sure there is enough tension in the rope and that the knot will not come untied. Tighten the knot by applying weight vertically.
