| A worker fell and was injured when a trucker’s hitch came untied. |
A worker used a trucker’s hitch to secure hydraulic oil drums to a service vehicle with a rope. When he pulled hard on the rope, the head of the knot slipped, causing the worker to lose his balance, fall over backward, and land hard on his back. |

| Corrective Action | |
| 1. | Tie a trucker’s hitch properly with a head loop 5 cm. in diameter. |
| 2. | Before pulling a rope at full strength, make sure there is enough tension in the rope and that the knot will not come untied. Tighten the knot by applying weight vertically. |
