A worker inadvertently stepped into the turning radius of a hydraulic shovel during a rotation test, while he was preoccupied with recording the measurement results, and was hit by the rotating bucket.

After measuring the swing speed of a hydraulic shovel during a test, the operator began rotating the bucket to the right to measure the swirl flow. The operator suddenly realized that the other tester, who had become preoccupied with the swing speed measurements, had inadvertently stepped into the turning radius of the shovel. He tried to avoid hitting the other tester by turning the shovel in the other direction, but could not change direction in time. The tester was hit by the bucket.

Corrective Action
1. Machinery operators should ensure the safety of the surrounding area before performing a maneuver.
2. Determine ahead of time where testers will stand and take their measurements, and mark off the hazardous areas.
3. Be sure never to step into the turning radius of a machine.
