During work to remove the track center guard of a hydraulic shovel, a worker's hand was crushed between the guard and the wrench.

Workers were working together to remove the track center guard from a hydraulic shovel.
Worker A was supporting the head of the ratchet wrench from the bottom to make the work easier, while worker B pulled on the wrench in front of him to loosen the attachment bolt. When the bolt suddenly gave way, the wrench rotated, and worker A's right hand was crushed in the gap between the guard and the wrench.



Corrective Action
1. Avoid potentially dangerous activities (implement hazard prediction training).
2. Use the proper tools (an L-wrench).
3. Ensure that cooperative workers are all familiar with safety confirmations and signals
