During the disassembly of the track adjuster spring of a hydraulic shovel, the tailored jig broke, hitting a worker in the upper right chest area.

A worker removed the track adjuster spring assembly from a hydraulic shovel and worked with another worker to disassemble the spring assembly. When a welded section of the jig (which was tailored to the spring assembly) to which the spring assembly was secured came apart, it hit worker A in the upper right chest area.



Corrective Action
1. Use the designated jig specified in the regulations.
2. Ensure that cooperative workers are all familiar with safety confirmations and signals.
