While preparing to transport a hydraulic shovel, a worker's right hand was crushed between the bucket and the base of the fork.

To transport a standard bucket together with a hydraulic shovel that was already equipped with a slope-finishing bucket, workers shoved the standard bucket inside the attached bucket.
When both workers pushed the bottom of the standard bucket, which was hanging from a Reach-Fork, the bucket swung and the momentum caused worker A's right hand to be crushed between the bucket and the base of the fork.


Corrective Action
1. Perform a safety check before doing work involving unstable objects.
2. Be diligent about taking safety precautions when working with others.
